We are looking for an enthusiastic experienced Clinical Pharmacist to join our Clinical Pharmacy Team working for the Barnstaple Alliance Primary Care Network.  The Barnstaple Alliance Primary Care Network comprises of 4 Practices, Litchdon Medical Centre, Brannam Medical Centre, Queens Medical Centre and Fremington Medical Centre.

The role will focus on work relating to managing patients with long term conditions to work across all care interfaces in relation to medication related issues and to deliver key objectives. To work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.

Hours can be flexible between 25 hours - 37.5 hours a week to suit the right candidate.

Location of post will be at Brannam Medical Centre but will also work at the other 3 practices.

Salary £46,000 - £59,000 WTE, depending on qualifications and experience.
