We welcome new patients joining the practice. To register, you need to complete a Registration Form and a New Patient Questionnaire. You will then be registered with the GP who is 'taking on' in that month.
Both the Registration Form and the New Patient Questionnaire are available to download from the above "Registering Online" tab. Once you have completed both forms, please send or bring them into the surgery.
You will be given the name of your own GP when you register so that you can see them regularly, however you may ask to see another GP or may be offered another GP for urgent appointments or 'same day team' appointments.
To fully register with the practice you need to live within approximately a 10 mile radius of the surgery.
Out of Area RegistrationsThere is an option if you live further away but wish to register with this practice and this is called 'an out of area registration'. This means you can register with us and attend here for appointments, however we are unable to carry out home visits. Please read this leaflet for more information OOAR V1 Jan 15 .pdf

Please complete both forms and ensure you add your NHS number and signature in the boxes required before you send the forms into us. We ask you to complete the New Patient Questionnaire as it can take some time for us to receive your medical records.Unsure what your NHS number is?Your NHS number is a 10 digit number which is unique to you. It will be on any letter or document you have received from the NHS such as; prescriptions, test results and appointment letters. If you don't have any of these documents to hand you can contact your GP practice and ask them to look it up for you.

If you are on holiday and need medical advice from a GP Surgery because you are unwell or have forgotten your medication please contact your own GP from where you live and they can help you - all practices are now doing telephone or video consultations and they can also send prescriptions electronically to anywhere in the country. This means your own GP, who knows you and your medical history, is in a better position to help you than being seeing as a temporary resident in a local surgery.
If you contact your own GP and they say you do need to be seen by a local GP you can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient please contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
